14.3 Assigning a Form to a Patient

 

A wide variety of forms is available in the ICE Health Correspondence module. Please see 2.8 Managing Forms Content to select the forms you would like to include in your office’s Forms database.

Please see 2.9 Auto-Assigning Presentations, Animations and Forms to auto-assign common forms to all new patients.

  • 1. Click the Correspondence icon  in the ICE Dental menu bar.
  • 2. Click Assign Form in the Correspondence tab.

  • 3. A Forms dialog box will list several Folders  that contain a variety of forms. Click the triangle expand button to list the contents of a folder.

  • 4. Select a forms checkbox  to assign any of the forms to the patient. A black square  in the folder checkbox will indicate that a form has been chosen from within that folder. Please note that if a folder checkbox  has been selected, all of the forms in that folder will have been assigned to the patient.
  • 5. Click OK to return to the Correspondence tab.
  • 6. The forms will now be listed in the patient’s files (beside a Form icon ) and the PV (Visible to Patient) checkbox will be selected.

  • 7. To view the form, click View under the title. The form will open up in a new browser tab and will be initially available to fill out online.
  • 8. Your patient can fill out their form at your office and click  (at the bottom of the form). The file list will be updated to indicate the date that the form was completed. Alternatively, they can sign in and fill out the form from home. Another option is to print the form and have your patients fill out their information manually. Please see 14.5 Scanning Images or Documents to scan the completed form to the patient’s records.
